How to Grow a C-L Service: A Workshop Sponsored by the ACLP C-L Division Directors’ SIG

A national survey of C-L Directors demonstrated that many felt underprepared for the challenge of growing CL services, including demonstrating value to executive leadership and developing multidisciplinary teams. This workshop will empower participants with knowledge from field leaders who have successfully grown services, using case studies to illustrate concepts including business planning, value, quality, and strategic alignment. Thomas Heinrich, MD, will review a medical business plan's critical components. This discussion will focus on a recently created value plan to establish a complexity intervention unit at an academic medical center. The conversation will focus on linking the proposed service to established institutional strategic priorities and incorporating innovative tactics to build the case to invest in the plan. Priya Gopalan, MD, will discuss CL growth for large networks. As national healthcare trends move towards consolidation of systems, large-scale corporate systems will become a regular part of the landscape. Dr. Gopalan will discuss the University of Pittsburgh Medical Health System, a hospital system with over 40 hospitals that functions as a major academic medical center, a corporate, nonprofit health care entity, and a payor-provider. She will utilize a case study to discuss managing a large CL team that spans 9 hospitals, focusing on telepsychiatry, standardization of practice, and harnessing community and outpatient partners. Attendees will engage in an exercise conceptualizing an additional hospital to existing CL services at their institution, allowing for practical, take-home guidance. Brian Bronson, MD, will present the expansion of CL services through the lens of multidisciplinary growth. He will use a case study of continual services growth at his institution from an under-staffed traditional CL division to a robust program with psychiatrists, nurses, psychologists, social workers and behavioralists in various roles. Sheryl Fleisch, MD will present the rationale, development, and implementation of a unique value-based proposal for C-L psychiatry services for homeless individuals, resulting in an award-winning program. Alex Brooks, MS, MBA, will provide CL directors with an administrator's perspective on how to grow a clinical service, including developing a service expansion proposal that will win the attention and financial support of hospital executive leadership.

Brian Bronson
Vice Chair for Clinical Strategy & Director of C-L Psychiatry | Stony Brook Medicine
Brian Bronson, MD is an Associate Professor and Vice-Chair for Clinical Strategy in the Department of Psychiatry at Stony Brook Medicine. He also serves as the Director for the Division of Consultation-Liaison Psychiatry and the Fellowship Program in CL Psychiatry. Prior to joining Stony Brook, he served for nine years on the faculty of New York University School of Medicine and the VA New York Harbor Healthcare System, where among other roles he led a VA grant-funded regional primary care mental health integration initiative across numerous VA systems. His research interests include health services at the interface of psychiatry and medicine. Within ACLP, he founded and chairs the C-L Psychiatry Division Directors Special Interest Group.
Thomas Heinrich
Professor of Psychiatry and Family Medicine | Medical College of Wisconsin
Thomas Heinrich, MD, Professor of Psychiatry and Family Medicine at the Medical College of Wisconsin (MCW), completed a combined residency in psychiatry and family medicine. This was followed by a fellowship in consultation psychiatry at Massachusetts General Hospital. He is board-certified in family medicine and general adult psychiatry and holds subspecialty certification in consultation-liaison psychiatry and neuropsychiatry. Clinically, Dr. Heinrich remains active in consultation psychiatry, working on a busy academic psychiatric consultation service at Froedtert Hospital, a quaternary care academic medical center that serves as MCW’s primary teaching hospital. The consultation service specializes in the assessment and treatment of the diverse and complex presentations of medical, surgical, and neuropsychiatric illnesses in patients admitted to the general medical hospital. Administratively, Dr. Heinrich currently serves as Director of the Division of Consultation-Liaison Psychiatry and Executive Vice-Chair in the Department of Psychiatry and Behavioral Medicine at MCW. In addition, he is the Medical Director of Process Improvement for inpatient medical and surgical services at Froedtert Hospital and Physician Lead for the implementation of Froedtert and the Medical College of Wisconsin’s multidisciplinary behavioral health strategic plan. In these roles, he has collaborated with multiple stakeholders to develop and implement several innovative cross-disciplinary and inter-institutional clinical programs.
Priya Gopalan
Associate Professor of Psychiatry | University of Pittsburgh Medical Center, Western Psychiatric Hospital
Dr. Gopalan is Associate Professor of Psychiatry and OB-GYN, Chief of Psychiatry for Magee-Womens Hospital, and Medical Director of the Psychiatry Consultation-Liaison service across 9 of the hospitals in the UPMC system. She has been active in programmatic development around CL psychiatry, perinatal mental health, and subspecialty integrated care. She is a faculty co-director to the psychiatry residency's Academic Administrator Clinician Educator (AACE) track and the Women’s Mental Health Area of Concentration at Western Psychiatric Hospital. Her interests include the administrative/business side of CL psychiatry, perinatal psychiatry, addictions, and trauma-related disorders.
Sheryl Fleisch
Chief of Psychiatric Services | University of Florida (UF) Shands Health Hospital
Dr. Fleisch is the chief of psychiatric services at the University of Florida (UF) Shands Health Hospital and chief of health equity in the Department of Psychiatry at the UF College of Medicine. She provides clinical care and leadership in the general hospital through the Psychiatry C-L service in the emergency room, and through a psychiatry-medicine unit. She also participates in the educational mission of UF as a teacher and mentor for medical students and residents. From 2013-2020, Dr. Fleisch served as founding medical director of the Vanderbilt University Medical Center (VUMC) Homeless Health Services and Street Psychiatry Programs. Her team provided psychiatric consultation services to people experiencing homelessness both in the hospital and on the streets of Nashville. While at VUMC, Dr. Fleisch also served as director of the C-L Fellowship Program.
Alex Brooks
Director of Operations | UPMC Presbyterian and Montefiore Hospitals
Alex Brooks, MS, MBA is currently the Director of Operations for Surgical Services at UPMC Presbyterian and UPMC Montefiore hospitals. In this role, Alex is primarily responsible for working with the Sterile Processing Department, Anesthesia Tech’s, the billing audit department, as well as leading other projects related to efficiency and process improvement for surgical services at the two hospitals and their 47 operating rooms. Prior to this role, Alex served as the Director of Operations and Special Projects at UPMC Western Psychiatric Hospital, the region’s leading behavioral health hospital and was principally focused on expanding access to earlier behavioral health interventions through the growth of UPMC's integrated behavioral health network. Alex is a 2020 graduate of UPMC Administrative Fellowship program and now serves as one of the co-directors of the program as well as the program director for the UPMC Administrative Summer Residency. Alex holds bachelor’s degrees in finance and business law from University of Maryland and master’s degrees in entrepreneurship & design and business administration from University of Delaware. Alex also currently serves as the treasurer and vice president of the Pittsburgh chapter of the National Association of Health Services Executives.
