Advances in Cardiac Psychiatry: An Interactive Workshop

The field of cardiac psychiatry is constantly evolving. In a recent scientific statement, the American Heart Association acknowledged the relationships between psychological and cardiovascular health and emphasized the importance of considering psychological health when treating individuals with heart disease. At the same time, advances in the diagnosis, monitoring, and treatment of cardiovascular disease—including the development of minimally invasive procedures and remote cardiac monitoring—have the potential to significantly impact mental and physical quality of life in patients with heart disease. Finally, in the past year, the cardiovascular and psychiatric effects of COVID-19 have become increasingly apparent. Knowledge of these advances is critical for C-L psychiatrists, as they strive to provide the most appropriate, evidence-based care for patients with heart disease. In this 90-minute workshop, we will provide a practical, evidence-based update on several essential topics for CL psychiatrists. These include advances in cardiology diagnosis, monitoring, and treatment, the cardiovascular and psychiatric consequences of COVID-19, psychiatric complications of cardiac transplant medications, approach to patients with complex cardiac disease in the setting of opioid use disorder, and psychiatric treatment considerations for patients with heart disease. In the first 60 minutes of the workshop, several presenters will provide updates on essential topics for CL psychiatrists. Dr. Styra will discuss new cardiac procedures and devices, including transcutaneous valve replacement and mitral clip procedures. Dr. Pozuelo will review recent advances in remote monitoring and wearable devices that can aid in diagnosis of cardiovascular disease and engagement in healthy behaviors. Dr. Funk will discuss cardiac complications of COVID and the approach to patients with complex cardiac disease in the setting of opioid use disorder. Dr. Celano will provide an update of the relationships between depression, anxiety, and heart disease, as well as treatment considerations for depressive and anxiety disorders in patients with heart disease. Finally, Dr. Saad-Pendergrass will review the psychiatric and neurologic side effects of cardiac transplant medications. In the final 30 minutes, we will hold an interactive panel discussion that will focus on the incorporation of this information into clinical care. Using several case vignettes, we will discuss the practical application of the topics presented to clinical practice, and audience members will be asked to provide feedback on how they would approach different clinical scenarios. We hope that attendees will leave this workshop with an increased understanding of the clinical implications of the technological advances in cardiology, the impact of COVID-19 and opioid use disorder on mental and cardiovascular health, and evidence-based treatment approaches to psychiatric disorders in patients with heart disease.

Christopher Celano
Associate Director, Cardiac Psychiatry Research Program | Massachusetts General Hospital
Dr. Celano is an attending psychiatrist and clinical researcher at the Massachusetts General Hospital. After finishing his CL fellowship at MGH in 2012, Dr. Celano shifted his focus to performing research on the intersection between psychiatric and cardiovascular health. His work focuses on the promotion of well-being and health behaviors in patients with cardiovascular disease, diabetes, and other chronic conditions, as well as the use of collaborative care interventions in patients with acute medical illness. He is the author of over 80 articles and book chapters and serves as the Associate Director of the Cardiac Psychiatry Research Program at MGH and as an associate editor of General Hospital Psychiatry, a peer-reviewed journal that focuses on inpatient, emergency, and consultation psychiatry.
Rima Styra
Director of the Division of Consultation-Liaison Psychiatry | University of Toronto
Dr. Styra is a staff psychiatrist at the University Health Network with a special interest in the area of cardiac interventions, both minimally invasive procedures as well as the cardiac and vascular surgery patient. She is the Director for the University of Toronto Department of Psychiatry Consultation Liaison program and the Director of the Behavioral Medicine program for the University of Toronto Physician Assistant program.

Hired in 1997, Leo Pozuelo is a staff psychiatrist at the Cleveland Clinic and has served as Section Head of Consultation Psychiatry from 2005-2019.
He is dual trained in internal medicine and psychiatry. He is a Fellow of the American College of Physicians and a Fellow of the Academy of Consultation Liaison Psychiatry.
He has served as psychiatry residency director and as medical school psychiatry clerkship director.
He maintains a behavioral cardiology clinic.
In 2011, he was appointed Vice Chair, Clinical Operations, at the Department of Psychiatry and Psychology of the Cleveland Clinic.
He collaborates with Cleveland Clinic Community Care (4Cs) in strategic development and hiring of mental health professionals to serve primary care patients.
In 2017, he completed a Health Care MBA from Baldwin Wallace University.
He took over the Consultation Liaison Psychiatry fellowship in 2019.
Also in 2019, he was elected to Board of Governors (BOG) of the Cleveland Clinic, which advises the CEO and Chief of Staff.

Margo Funk
Director of Cardiovascular Psychiatry, Program Director Adult Psychiatry Residency | Brigham and Women's Hospital
Dr. Margo Funk is the Director of Cardiovascular Psychiatry and the Program Director of the Adult Psychiatry Residency at Brigham and Women's Hospital. Dr. Funk started her career at the Cleveland Clinic as a staff psychiatrist on the C-L service, the Associate Program Director of the Adult Psychiatry Residency, and served in multiple medical student education leadership roles. Dr. Funk then joined the VA where she was Chief of Psychiatry, and later Associate Chief of Staff for Mental Health at the Southern Oregon VA. She transitioned to be Program Director of the Harvard South Shore Adult Psychiatry Residency at VA Boston prior to moving to BWH this past summer. Dr. Funk was Chair of the APA Council on CL Psychiatry's workgroup on QTc Prolongation and Psychotropic Medications and currently chairs the APA workgroup on Treatment of Opioid Use Disorder in the General Hospital. Dr. Funk's interests include cardiovascular psychiatry, medical education, and advancement of women in healthcare leadership.
